Connecticut Children’s Medical Center: Innovation Grant
Funds were granted to further test implementation of co-management protocols. The project addressed shared care between pediatric subspecialists and primary care pediatricians for management of concussion and obesity.

Discovery Communities: Early Childhood Community Grants
Funds were granted to support 14 communities to integrate health strategies into their early childhood plans and to support plan implementation. Funds were also provided to the communities by the William Caspar Graustein Memorial Fund and the State Department of Education.

Funds support the United Way in bringing together care coordination services for the Hartford regions from three state agencies to better serve families.

ProHealth Physicians: Psychotropic Medication Initiative
Funds were granted to support the development, implementation and evaluation of an algorithm for managing anxiety and depression in pediatric primary care.

UConn Health Center, Department of Psychiatry: Psychotropic Medication Initiative
Funds were granted to develop a demonstration project to increase the capacity of three pediatric primary care sites to detect anxiety and depression in children and work with mental health providers to address care.

Funds were granted for a comprehensive literature review and report.

Funds were granted to support the development, implementation and evaluation of an algorithm for managing anxiety and depression in pediatric primary care.

The President’s Discretionary Fund was used to provide grants to support Medication Administration project consultants, the Connecticut Family Support Network, Connecticut Oral Health Initiative, and the Connecticut News Project.
